CASE MANAGER
SUMMARY: The Case Manager provides intensive, individualized support to women experiencing homelessness, guiding them through a strengths-based, trauma-informed process toward permanent housing, improved health, and greater stability. Working as part of an integrated services team, the Case Manager conducts assessments, develops housing and service plans, connects clients to resources, and provides ongoing coaching and advocacy. This role requires deep knowledge of housing-first principles, local resources, and the unique challenges facing unhoused women.
QUALIFICATIONS:
KEY RESPONSIBILITIES
Client Engagement & Case Management
· Conduct comprehensive assessments to determine client needs, strengths, and housing barriers
· Develop individualized service plans in collaboration with each client to achieve housing stability and personal goals
· Provide direct support and regular check-ins to help clients navigate housing applications, benefits, mental health care, employment, and other services
· Assist clients with accessing documents (ID, SS card, birth certificate) and housing-related paperwork
· Maintain consistent, culturally responsive, and trauma-informed engagement with clients, building trust and rapport
Housing Support & Navigation
· Work with clients to identify individually appropriate housing options
· Coordinate with housing navigators, landlords, and external partners to facilitate move-in, lease signing, and tenancy support
· Conduct housing searches, unit inspections, and accompany clients to appointments as needed
· Support clients in maintaining housing and preventing eviction through proactive communication and problem-solving
Documentation, Compliance, and Reporting
· Maintain accurate, timely case notes and documentation in accordance with funder and organizational requirements
· Complete all required assessments and data entry in HMIS and other internal case management systems
· Contribute to monthly and quarterly program reporting by tracking client outcomes and service benchmarks
Team Collaboration & Professional Development
· Participate in regular case conferencing, supervision, and interdisciplinary team meetings
· Attend trainings on trauma-informed care, harm reduction, equity, and housing-first practices
· Support crisis response efforts and cross-team collaboration (e.g., Respite, Day Shelter)
QUALIFICATIONS
· Bachelor’s degree in social work, psychology, or a related field; or equivalent experience working in direct service settings with marginalized populations
· At least 2 years of experience in case management, housing services, or behavioral health preferred
· Strong knowledge of community resources, housing programs, and systems navigation in Wake County, NC
· Familiarity with trauma-informed care, harm reduction, and housing-first models
· Excellent interpersonal, communication, and organizational skills
· Ability to maintain professional boundaries and uphold confidentiality
· Proficiency with case documentation and data entry; experience with HMIS a plus
· Commitment to dignity, and inclusive support for women, including LGBTQ+ and BIPOC communities

Working Conditions
· Based at The Women’s Center facility with occasional travel in the community
· May involve home visits, medical appointments, or accompanying clients to housing-related services
· Occasional evening or weekend hours may be required
· Must be able to lift 15 lbs and work in a fast-paced, emotionally demanding environment
HOURS: Generally, 7:45am – 3:30pm weekdays with some occasional weekends and evenings.
SALARY: This is an salaried position with a hiring range of $40,000 - $45,000. Benefits include: Health, vision, and dental insurance at 80% employer coverage.
